Nitro 1 Sports Medical Release 

RELEASE AND LEGAL AGREEMENT 

I am the participant named on the front of this form and below (Participant) or the parent or  legal guardian of the Participant. I hereby agree to the following. 

1. I acknowledge that participation in athletic events necessarily involves risk of physical injury.  In return for Nitro1Sports LLC,  accepting the registration of the Participant and permitting his or her the voluntary participation in its programs, I hereby  release, discharge and hold harmless Nitro1Sports LLC, its owner, directors, employees, team leaders, assistant team leaders, volunteers and other representatives from any claims arising out of or relating to any injury that may result to the Participant in connection with Nitro1Sports LLC sponsored events, including games and practices, including any injury caused by the negligence of any team leader, assistant team leader, while performing his/her duties during any events.  

2. I hereby authorize any representative of Nitro1Sports LLC,  to act for me in a medical emergency. I  agree to indemnify and hold harmless Nitro1Sports LLC, its owner, directors, employees, team leaders, assistant team leaders, volunteers and other representatives from any claims, including claims relating to the  payment of medical bills or fees, arising out of or relating to any medical treatment duly  authorized by any Nitro1Sports LLC representative. 

WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19 -Nitro1Sports LLC   ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT 

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by  the World Health Organization. COVID-19 is extremely contagious and is believed to  spread mainly from person-to-person contact. As a result, federal, state, and local  governments and federal and state health agencies recommend social distancing and  have, in many locations, prohibited the congregation of groups of people. 

 

Nitro1Sports, LLC has put in place preventative measures to reduce the spread of COVID-19; however, Nitro1Sports cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Nitro1SportsCamp could increase your risk and your child(ren)’s risk of contracting  COVID-19.  

By signing this agreement, I acknowledge the contagious nature of COVID-19  and voluntarily assume the risk that my child(ren) and I may be exposed to or infected  by COVID-19 by attending the Nitro1Sports Camp and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 during the Camp may result from the actions, missions, or negligence of myself and others, including, but not limited to, Camp employees, volunteers, and program participants and their families. 

I voluntarily agree to assume all of the foregoing risks and accept sole  responsibility for any injury to my child(ren) or myself (including, but not limited to,  personal injury, disability, and death), illness, damage, loss, claim, liability, or expense,  of any kind, that I or my child(ren) may experience or incur in connection with my  child(ren)’s attendance at the Camp or participation in Camp programming (“Claims”). On  my behalf, and on behalf of my children, I hereby release, covenant not to sue,  discharge, and hold harmless the Camp, its employees, agents, and representatives, of  and from the Claims, including all liabilities, claims, actions, damages, costs or  expenses of any kind arising out of or relating thereto. I understand and agree that this  release includes any Claims based on the actions, omissions, or negligence of the Camp, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Nitro1Sports Camp. 

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT,  FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP  SUBSTANTIAL RIGHTS BY AGREEING TO IT, AND AGREE FREELY AND VOLUNTARILY  WITHOUT ANY INDUCEMENT.
